New Applicants
New MedNet applicants should locate their provider type and access the list of required forms to begin the application process. These forms must be completed and mailed to MedNet’s Credentialing Department in order to begin the enrollment process.
As of October 1, 2011 and until further notice, MedNet is not accepting applications for the following Mental Health Provider Specialties.
- Licensed Clinical Social Worker
- Licensed Clinical Professional Counselor
- Licensed Alcohol and Drug Abuse Counselor
- Licensed Marriage and Family Therapist
- Licensed Pastoral Counselor
