- National Provider Identifier (NPI) – update your NPI in the way that is most convenient for you:
- Visit UnitedHealthcare Online (UnitedHealthcareOnline.com) to update your NPI and related information online, login with your User ID and Password. Go to “Practice/Facility Profile” and select your TIN. Click “continue”, then select the “View/Update NPI Information” tab.
- Fax your NPI to the appropriate fax number based on your geographic location/state. Download/Print the fax form.
- Call 877-842-3210, the Enhanced Voice Portal. Select the “Health Care Professional Services” prompt. State “Demographic changes” and your call will be directed to the Service Center to collect your NPI, corresponding NUCC Taxonomy Codes, and other NPI-related information
- Your UnitedHealth premium assessment report
